If your student has a medical condition that requires attention or accommodation during the school day, this page will help you find the right care plans and forms to submit to your school nurse. Many conditions require a care plan completed and signed by your medical provider. Many of these must be updated each school year. Allergies
Please provide the care plan completed and signed by your medical provider. ASD20 requires updated plans each school year. If you want your student to carry their epipen (building administration approval required; not allowed in elementary schools), ensure your provider marks the appropriate box on the care plan, fill out the self-carry contract, and return both to the school nurse. It is highly recommended parents provide a back-up epipen for the health room. Any medications not listed on the care plan require a Permission to Administer Medication form.

Asthma
Please provide the care plan completed and signed by your medical provider and the asthma intake form. ASD20 requires an updated plan each school year. If you want your student to carry their asthma inhaler (building administration approval required; not allowed in elementary schools), ensure your provider marks the appropriate box on the care plan, fill out the self-carry contract, and return both to the school nurse. It is highly recommended parents provide a back-up rescue inhaler for the health room. Any medications not listed on the care plan require a Permission to Administer Medication form.

Center-Based Student Emergency Information Forms
Please fill out the appropriate form for your student in a center-based program.
Diabetes
Please provide your student’s Diabetes Medical Management Plan from their endocrinologist to the school nurse. Additionally, complete the Self-Management of DM in the School Setting form with your student and provider, if applicable. Contact your school nurse the week before school starts to drop off emergency medications, back-up supplies, and snacks.
Headaches
Please provide the care plan completed and signed by your medical provider. ASD20 requires an updated plan each school year. If you want your student to carry their own headache medication, fill out the General Self Carry Contract (no controlled substances; not allowed in elementary schools). Any medications not listed on the care plan require a Permission to Administer Medication form.
How Sick is Too Sick
ASD20 schools follow Colorado Department of Education guidelines on when to exclude a student from school due to illness.
Immunizations
Colorado law requires all students attending Colorado schools and licensed child care to be vaccinated against certain diseases unless a medical or non-medical exemption is filed. Visit our Immunization Requirements page for additional information.
Meal Modifications
