Immunizations are an important part of health care. Colorado law recommends children attending childcare or preschool are vaccinated to prevent vaccine preventable diseases. Academy School District 20 requires all students meet the immunization requirements established by the Colorado Board of Health. Students who do not meet the requirements may be denied attendance according to Colorado Revised Statutes 25-4-902.
Proof of immunization or exemption forms (or otherwise as noted below), or letters from parents or doctors, must be provided at the start of each school year. For an exemption, a Medical Exemption form signed by a health care provider or a Non-Medical exemption (religious or personal) may be submitted on the form located in policy JLCB E2 of via a letter from a parent or doctor.

Additionally, the CDPHE recognizes that some families may prefer not to use a form, a parent/guardian or emancipated student or student over the age of 18 may submit a signed non-medical statement of exemption per section 25-4-903(2)(b), C.R.S. Such a statement should include the following information: student's full name, age or date of birth, date the exemption was submitted, the vaccines declined, and which type of non-medical exemption is being taken (personal belief or religious).
