  2009-2010 Immunization Requirements


This year, Eagleview's TAKING FLIGHT will take place on Monday, August 10 and Thursday, August 13.  At
TAKING FLIGHT, you will be able to:

  • Have your student's school picture taken
  • Complete your student's tech agreement
  • Receive your student's ID card
  • Receive and pay for your student's planner (and atlas for 6th grade)
  • Order and pay for a yearbook
  • Sign up and pay for your student's participation in fall sports and complete/turn-in the necessary sports' paperwork
  • Pick up your student's schedule and locker assignment
  • Submit updated copies of Immunization records
  • Buy Eagleview Spirit Wear at the student store (separate payment needed)
  • Pay on your student's lunch account (separate payment needed)


NEW THIS YEAR!
Students will have their school pictures taken during TAKING FLIGHT this year!  Please make sure to plan accordingly.  For order forms please click here or visit the Quick Links to Forms link in the left navigation bar.

Approve your 2009-2010 Student User Agreement before you come to TAKING FLIGHT via your Parent Portal from home. Go to the district web site www.asd20.org/homeinfo click on "Sign Student Network Agreement Electronically" in the center of the page. If you forgot your password you will find a link to reset your password on the Parent/Student main page. Please complete before TAKING FLIGHT.


Upon completion of TAKING FLIGHT, we encourage you to allow your child to organize their locker, try out their combination, and find their new classrooms.  Please be aware that our teachers will be receiving training during this time and will be unavailable.

To access pertinent forms (including sports physical forms and school picture forms), please use the Quick Links to Form located on the EMS main page.  Please bring all completed forms to
TAKING FLIGHT. Only the yearbook form will be available at
TAKING FLIGHT.

Eagleview Middle School is pleased to offer EduKit school supply kits for the upcoming school year as a convenient service for our busy families!  The kits are custom-made to match the specific supply lists requested by our teachers.  Edukits contain all NAME BRAND products, competitive prices, all at no stress for you.  Your child's EduKit can be purchased online until July 20th and will be available for pick-up at
TAKING FLIGHT at the second service window by the Student Office entrance.  Please use the following link or the Edukit link on the EMS main page:

If you are unable to attend a
TAKING FLIGHT day, you have the option of completing and printing the forms including the Mail-in payment form and mailing them to Eagleview Middle School, 1325 Vindicator Dr., Colorado Springs, CO 80919.  Please mail your forms and payment no later than August 3, 2009.  You will be able to pick up your supplies etc. from the second service window outside the Student Office on the first day of school.  The 6th graders first day of school is August 17th (7th and 8th grade students will start on August 18th).
